Why our Doctors ? Why at home ?

Imagine a world where healthcare comes to your doorstep, where compassionate doctors cater to the unique needs of individuals with both mental and physical special needs.


This medical group manages and supports a significant number of patients with special needs, their families, and also support the other specialists for those patients. 


Everything we do here starts and ends with the same question: "how is this best accomplished remotely, at home in the patient's own environment?”


Sometimes families just need blood draws done at home or vaccines given for a patient who doesn't travel easily. Our MA phlebotomists have extensive experience making this easy and comfortable — all done at home, in the one place your family member is most comfortable


Autistic, physically disabled or developmentally disabled.   The HCDTO team of dedicated professionals specializes in caring for patients who require extra attention and understanding.

Doctors manage complex needs

Occasionally the medical issues are more difficult, including managing patients with:

 

  • Missing chromosomes or severe genetic abnormalities
  • Duchennes muscular dystrophy, MS, ALS, Huntington's and other major diseases
  • Auto immune cases, 
  • Complex cases following major  organ failure

 

Our medical group has extensive experience with IV antibiotics administered in the home.  Even better we are typically thinking two and three steps ahead of the standard insurance company or Hospital answer.


Emergency situations 

Calling an ambulance is an area when thinking ahead matters. If our doctors recommend a patient go to the ER -- we go a step above and beyond.  


Our docs will call the ER – calling BEFORE the ambulance arrives at the hospital – to discuss the case coming in and the patients critical care issues.   This extra step makes a world of difference in getting the right care underway quickly with a better understanding at the Emergency Department.   


Many of our providers also work in the ERs throughout this region. The hospitals know when an HCDTO patient is coming in that we demand good communication and admission to the better hospitalist teams at each hospital.

Recurring agitation at home

Occasionally there is an easier answer:   

We have any number of patients with developmental disabilities who are unable to verbalize when they are itching, uncomfortable or have something irritating them. You may have seen your loved one scratching or pounding at their own ears to express their frustration. 


Our medical staff has found an easy non-invasive procedure can be done at home that some non-verbal patients find soothing. 


Call and ask why a number of our autistic and other families schedule our medical staff for a short appointment at their home.  We may be able to reduce the times when your child is beating/scratching his or her own ears and face.

One of the best examples

A complex patient of ours was in the hospital and needed IV antibiotics. The hospital’s answer was discharge to home and administer “Brand X” as an IV antibiotic three times per day for two weeks. Trying to get that done at home is a massive challenge under both insurance and skilled nursing resources.


Instead our medical group went back to the hospital and the insurance company and proposed a different antibiotic that only had to be given JUST ONCE every seven days. That's only twice at home. Easy to do. Easy to manage. 


Our doctors have the experience — and the time — to think several steps ahead.

Planning for transport or surgery

Sometimes medical needs require complex planning. Several patients have needed oral surgery under moderate sedation — the problem is HOW to transport an autistic person to the surgical clinic. 


More than one government agency and family have called us for help. 


After medical clearance the: 

  • MD met everyone at the patient’s home, 
  • administered oral anti-anxiety meds, 
  • Doctor walked down the stairs with the patient’s hand on the MD’s shoulder (and a parent behind),
  • then the MD drove following the government vehicle to the oral surgeon’s office,
  • administered other medications to safely get the patient into the surgeon,
  • and at the request of the parent and government agency performed the pt’s first actual physical in over 15 years prior to surgery.
